[backport server-21.1] Initialize Mode->name in xf86CVTMode()

A bit silly as this obseletes !1309 (merged) and !1307 (merged) (they still might be useful in other similar issues though). Sergiy on the freebsd bugzilla figured out that server-21.1 did not set CVT mode names for VESA FlatPanel devices on xf86-video-nv, oops!
