Drop ifdefs for ABI_VIDEODRV_VERSION < 12. ABI_VIDEODRV_VERSION 12 shipped in Xserver 1.10, less than our new minimum. Mostly removes PC98 support
Remove XAA support. XAA support was removed in xserver-1.13.0 (released in 2012).
