Default xterm window has no header: can't be moved or resized

Description

On launching XQuartz 2.7.11, when the xterm window appears, there is no header bar, so there is no window title or the three buttons on the top left to close, maximise and minimise the window. This carries across to all other windows created when running X11 apps from a remote machine. This started happening yesterday: prior to that XQuartz had been running fine for months. Have tried doing a complete re-install of XQuartz and deleting preference files but nothing fixes it.

Version: 2.7.11 (xserver-1.18.4)
