make "Both Alts" work again, add an "Alt+AltGr" option, and then clean up
The first patch restores the alts_toggle to working order.
(If there is still a desktop environment that automatically enables a group toggle when the user selects more than one layout, then this is a bug in the DE, not in xkeyboard-config. Xfce and Mate at least don't do this -- the user has to pick a toggle explicitly, or use an applet in the system tray.)
The second patch adds an alt_altgr_toggle for use with multilevel layouts.
And the third patch cleans up stuff that is no longer needed.
