improve various comments, and delete some redundant lines
The first patch removes two key definitions that get overridden by another definition in the same layout.
The second and third patch remove some redundant comments and whitespace.
The fourth patch replaces the deprecated leftcaret and rightcaret symbols.
The fifth patch deletes four obsolete comments.
The sixth patch correct a mistaken caret name to asciicircum in a Mac layout.
And the seventh patch improves seven comments about the effect of Mode_switch.
