Remove some useless includes, comments, and blank lines
The first patch removes four pointless include statements from the US layouts. The second patch removes some comments that were copied from elsewehere but don't really apply any more, and condenses some stuff by putting comments at the end of the lines that they refer to, as is the custom. And the third patch mainly removes some pointless trailing blank lines in a bunch of layouts.
