Add Issue template

This MR introduces a default issue template that will people reporting issues remember all the standard questions that will get asked anyway (desktop environment, version) and gives a little guidance on how to use code-blocks as I've seen that we have people not familiar with markdown reporting bugs, resulting in not very readable issues.
