Fix Issue 243: Proper quoting of `_MANUALPAGE` and `_USAGE` sections.

This adds proper quoting for the documentation sections to resolve #243 (closed) and make sure it doesn't happen again.

I've also taken the liberty of fixing the slightly misleading wording in the trouble causing section of the documentation.
