layer-shell: Bind to n-th output and add help

Bind to the n-th output global rather than the first one seen

Fixes: 295dba69 ("Update layer shell example -o behavior")

while at that add help output on -? (I don't use layer-shell client very often and I always need to look up the options in the source code otherwise 😇)
