vrend: take start_instance in instanced drawing into account

Merged Gert Wollny requested to merge gerddie/virglrenderer:fix-base-instance-plus-base-vertex into master

We ignored the possibility of having both, base instance and base vertex, and we also ignored that it is possible to draw just one instance with a start instance > 0. This MR fixes this.
