doc: Homogenize Markdown formatting

• SPDX in all files
• Always a link to the front page
• ToC where appropriate
• 2 newlines before Sections of Level 1 or 2 (1 newline for >= 3)
• 80 characters line limit
• Use of reference-style links where appropriate to not break the flow
• link references always at the end of a Section
