Monitor fixes

Julian Bouzas requested to merge julian/wireplumber:monitor-fixes into master

Like pipewire's media session, this MR enables monitor ports in the nodes by default, which is needed so that tools such as pavucontrol can monitor audio. The policy scripts have also been improved to work with client nodes that want to be linked with other client nodes (both with media class Stream/*/*).
