pulse-server: fill in active port for monitors

Without setting the active port for monitors, collect_device_info will always use the fallback which assigns the first port as active. This results in pulse clients like firefox thinking the monitor is unplugged and wont allow to use it as an audio source if the fallback port is unplugged.
