nvk: Clean up some pushbuf stuff

Faith Ekstrand requested to merge gfxstrand/mesa:nvk/cmd-buffer-chain into nouveau/vk

This is mostly syntactic cleanups and dropping unneeded BO referencing. Gets rid of all instances of cmd->push inside a P_* macro.
