Draft: Set modem state to failed if lock status cannot be read

Pavan Holla requested to merge pholla/ModemManager:puk_fail_1 into master

If the lock status cannot be read during a pin/puk unblock attempt, move the modem to a failed state. This is consistent with the behavior observed if a lock status read fails during initialization.
