shared-qmi: detect eSIM profile switches on most QMI modems

Teijo Kinnunen requested to merge teijo.kinnunen/ModemManager:qmi_prof_switch into master

Use either physical slot status indications (if supported by the modem) or REFRESH notifications to detect when ICCID of the SIM card has changed. Trigger SIM hot swap mechanism if ICCID change is detected.
