intel: add driver identifier BO

We're getting some amount of hang reports from users and my first question always is : What version of Mesa are you running?

Adding a 4Kb BO to all execbufs seems like a small price to pay so that we get this information automatically in the error state.

This series only adds the driver name + Mesa version, but we could easily add the drirc options we're running with.
