fmp4/mp4: initial implementation of flac in mp4

Still a very initial implementation of storing flac in mp4 and fmp4. Based on information in isoflac.txt and my own experience playing these files to a phone and such.

Remaining things:

  • Tests?
  • In-line TODOs/comments
  • Extend this to regular mp4mux?

Creating this initial draft PR to gather feedback on approach and to have something concrete in hand when asking questions.
