fallbackswitch: fix and prevent deadlocks

I'm trying to drain fallbackswitch's mutexes swamp.

  • ensure strict ordering when taking mutexes
  • prevent deadlocks in chain function
  • implement the stream lock protection suggested in #202 (comment 1387170)
