Splitmuxsink muxerpad map

Add a muxer-pad-map property to splitmuxsink that allows explicitly mapping input sink pads to the corresponding muxer pads, bypassing the automatic splitmuxsink behaviour that creates muxer pads on a first-come-first-served basis.

Also a couple of docs fixes, and a fix for async-mode that ensures streams are consistent across fragments.
