User verification: shorten the template
The current template is too verbose and apparently confusing to users, so let's shorten it to something that users will hopefully read and grasp in its entirety.
Also move the why up since that's the one we really want people to fill out manually, the rest is just checkboxes.
Related to ackuser!3, ideally makes that one no longer necessary.
