I have 2 digital devices, GK104 HDMI and an M5A99FX this happens to both, therefor the problem would be PA, logically. Alsa modules are compiled into kernel, i.e. they are not modules.
I have flat volumes disabled.
Case #1:
ALC892 Digital HDA ATI SB
In log.txt you'll find pluginloader.exe audio stream #1 (wine pipelight) is playing sound at 100%. I start a new stream from TESV.exe, SkyrimLauncher.exe (wine) unhooks AND THE SOUND GOES CRAZILLY LOUD. decreasing volume on pluginloader.exe audio stream #1 does not work, audio is same level of loudness until it hits 0%, putting the level back to 1% causes the audio to come on CRAZY LOUD level again.
Case #2:
HDA NVidia GK104 HDMI
No log, same situation, stream from vlc using pulseaudio, in kde I set gk104 HDMI as preferred over the mobo audio. While vlc is streaming I launch chrome and start a native pepperflash youtube video. Same problem as above.

I'm using Fedora 20/KDE with 3.13.5-200 and pulseaudio version 4.0. Whenever I record something or use the mic for Mumble there's a really bad static following the audio in the recording which prevents me from using the mic in a proper manner to talk to people or to record. This has been an issue for a while and I have not been able to resolve this. The funny part is if I'm using the pulseaudio daemon the static follows after a few seconds and if I'm not running the audio, it follows almost immediately. It lasts the same length of time as how long I speak so it seems like some sort of echo.

Steps to reproduce:
1. Use microphone or record
2. Audio stream will have really bad static.

I am running Fedora 16 on an Acer Aspire 1410. It has analog jacks for mic/headphone and a fullsize HDMI connector. I can thus output both video and audio to an HDTV.
Sometimes when I plug into HDMI and ask gnome-control-center to switch the hardware's profile from "Analog Stereo Duplex" to "Digital Stereo (HDMI) Output", it doesn't work. You don't actually get sound on the output, and if you click the "Test speakers" button gnome-control-center dies with as segfault.
Sometimes, it's the other way around. For example, this is what I recall has happened in the last 12 hours:
1. Plugged the laptop onto an HDTV's HDMI cable. Screen turns on automatically, and I disable the laptop screen.
2. In gnome-control-center, change the hardware profile from "Analog Stereo Duplex" to "Digital Stereo (HDMI) Output"
3. It works! Grab some popcorn.
4. Unplug the HDMI cable and forget to care about audio for a few hours.
5. Try to watch some youtube video. "Oh snap, it's still on HDMI audio"
6. In gnome-control-center, change the hardware profile from to "Digital Stereo (HDMI) Output" to "Analog Stereo Duplex". Reload the youtube tab, no joy. Close the browser and reload it, no joy.
7. Go to gnome-control-center's sound prefs, click the Test speakers button, segfault.
8. alt+F2, "pulseaudio -k && pulseaudio -D"
9. Hardware profile switching now works.

Why do I use system-wide mode although it is not recommended? I have two user sessions. One my private, one my work one. The music is on the private one. And I like to hear music with ear phones at work at times. The pulseaudio on my private session stops playing when I switch to the work session.

i am running archlinux x64 and using gnome. I have a LG 29EA93-P Monitor connected with display port and got the following issue:
The sound disappears very randomly, sometimes when i pause a video and then press play again, sometimes i play several musicvideos in my playlist and then the sound disappears. I think it happens when there is no sound activity in between.
When the sound is away i have to close the youtube tab or close the application, open the gnome-shell sound settings and click on test speakers left/right then i hear "eft" or "right" (but it should be "Front left/right") afterwards the sound works again.
Recently i figured out that "pulseaudio -k" also helps.
I tried to get some debug information, please look at the attachment. Let me know if i can help solving this! I struggle with this issue since more then a year.

When PulseAudio runs as a part of a session, the "exit-idle-time" value should be ignored, so that PulseAudio doesn't stay running after the session has ended.
I believe the exit-idle-time option exists only to handle the cases where PulseAudio is autospawned outside a proper login session. In those cases exit-idle-time is needed, because otherwise PulseAudio would either run forever or exit immediately after the first client exits, neither of which is good.

Steps to reproduce:
1. Setup new pulseaudio in system mode
2. Exec following:

Additional info:
Sometimes PA accepts second stream without any timeouts but I can't find any particular conditions in which it was happening.
While PA is in a hang I can still use HTTP/CLI protocols (pactl, Web UI).
Please note that PA system mode is because because I want to use my RPi device as a network media player.
I'm using Fedora 23 on RPi 2 and here are some details:

When I plug in the USB Sonica Theater and Pulseaudio recognized instead of the bailing out with "usb_set_interface failed" message I get the following situation regarding volume acording to alsamixer:
- left channel: 0
- right channel: 79
Expected results:
Both channels at 79 or something like that.
I reported this on Pulseaudio mailing list already:

I think its due to:

On one FreeBSD laptop mpv plays video much faster when pulseaudio is enabled (--ao pulse). With --ao oss the speed is normal. Clock itself there runs with the normal speed.
It appears that pulseaudio is what causes it, but I don't understand the mechanism. It is also unclear why the overall playing speed depends on the pulseaudio which only plays sound.

If DBus module is not loaded into PulseAudio, the DBus interface for server socket lookup is still available and the property Address returns a path to non-existing file.
IMHO, PA should not expose /org/pulseaudio/server_lookup1 object if the DBus module is unloaded.

From the XDG Base Dir Spec (v 0.7):
"If $XDG_RUNTIME_DIR is not set applications should fall back to a
replacement directory with similar capabilities and print a warning
message."
I understand that "should" clauses are not mandatory, but imo, it would be very helpful to have this particular warning issued, and to state explicitly the replacement directory that is being used instead. Even better would be to issue an informational message on every PA daemon startup that states the runtime directory. (The latter could perhaps be conditional on -v.)
