Reproduction (speakers are connected, also the headphone):
1. plug out headphones, the sound should be hearable from the speakers
2. listen to music some time
3. plug in headphones
Now this problem appears sometimes (with the headphone, the speakers still operate fine). The problem can't be solved by restarting pulseaudio.
The weirdest thing is that the problem disappears after a while.
Sorry that I don't have more information. I hope you can reproduce the problem.
my conclusion:
It seems as if the left and the right channel are mixed to a distorted mono sound.
But this seems only to happen in headphones. Most probably it is a fault in the headphone detection (the headphone itself is new and works fine) or a hardware problem (broken driver?)

When a sink or source belongs to a card, I think it would be a good idea to make pa_card the owner of the sink or source. This would mean that pa_card would decide when to create and destroy sinks and sources (they would be created at profile activation and destroyed at profile deactivation). I think this would make a lot of sense conceptually, but I'm not whether the effects on the back end complexity would be positive or negative (I would expect a positive effect).
The unclear part is how to delegate the back end specific parts of the sink initialization (and deinitialization) to the back end, if pa_sink_new() is called from card.c. I don't think we currently have any similar object initialization scheme in PA.
This idea occurred to me while implementing device prototypes. It's annoying that card back ends need to explicitly link the device prototypes with the sink or source whenever the back end creates a sink/source. It's only one line per created sink/source, but still it feels like unnecessary boilerplate code. The Linux kernel, for some time, has been having random problems with Logitech webcams. I have encountered this problem in Fedora, but there is a large community of Ubuntu users who have this problem also.

The basic issue seems to be that the kernel somehow messes up the webcam, and the webcam has to be unplugged and replugged in order to continue.
However, while PulseAudio is not the "root cause" of the issue, it appears to handle the problem in a sub-optimal way:
1) Once the webcam is "broken", all PulseAudio sound becomes broken, even to devices that are still working perfectly well.
2) Once the webcam is unplugged and replugged, PulseAudio needs to be restarted. It does not automatically recover from whatever situation it has gotten into.

When I enable ORC, start pulseaudio (either 8.0 or 9.0) and use pavucontrol to change the volume of my HDMI sink, pavucontrol and alsa-sink-HDMI segfault.

Description of problem:
Days ago I disabled PulseAudio's flat volumes due PulseAudio:
- crackling audio trouble (bugreport #92031)

Disabling PulseAudio's flat volumes solved the mentioned problems, but introduced a new one.
I usually increase / decrease volume level by using mouse wheel on KMix icon.
Both actions sometimes trigger the master volume to 100% level. While collecting PulseAudio logs, I managed to reproduce (twice) the problem:
Note: each mousewheel step increases or decreases volume level by a 5%.
===First occurrence of the problem===
At line number 1359 you can see the volume setted at 70%:
A few moments later, I tried to increase it at 75%, but the volume level has been pushed to 100% (line number 1459).
===Second occurrence of the problem===
At line number 18000 you can see the volume setted at 40%:
A few moments later, I tried to increase it at 45%, but the volume level has been pushed to 100% (line number 18016).
Since on the logs, every volume level change has the form of

that "Requested volume" makes me think about PulseAudio being requested to set the volume to 100%. So in the dubt I added in CC some guys of KDE SIG and alsa.
I can quite easly reproduce the problem, so feel free to ask me to do any test.

Result:
Every time you change the volume of one of: Notification Sound, or Audacious, there is is a popping noise.
Workaround:
Mute System Notifications Sound, there is no noise when chaining volume of Audacious.
Example2:
- You got Audacious playing music
- You have mumble running (no speaking just running idle)
Result:
There is always a noise changing volume of Mumble or Audacious!
Workaround:
Have only one app running at a time!
Because Mumble streams even if it is idle, it counts as second stream here. So there will be this noise every single time you have more then one stream.
Searching on Google about this got me thousands of site who say, just have one stream always and everything is fine.

When running PulseAudio, if I'm playing music via any source (Exaile, Totem, Flash in Firefox or Chromium, VLC, etc.) every few minutes the audio will skip once or twice. top doesn't show any process eating CPU any more when it happens. dmesg doesn't show anything. Nothing else seems to be affected. It doesn't seem to happen regularly.
I just installed Ubuntu 12.04 from scratch (with pulseaudio 1:1.1-0ubuntu15), but the same thing happened in 10.04 on the same hardware. Uninstalling the PulseAudio packages and going back to ALSA gives me no issues whatsoever, which is how I know the issue is with PulseAudio. I'm using my onboard audio device:

I've been trying to get Ubuntu/Kubuntu going on my system:
Desktop Dell 400sc, CPU Pentium 4 Dual Core,
RAM 4G Dual Channel, Video ATI All In Wonder 8500 (128M), MyGica A680B-W7 USB HDTV Stick, Pioneer DVD-ROM/CD Burner, Turtle Beach Santa Cruz Sound Card, Logitech C510 WebCam.
And am having trouble with the sound. I tried both Ubuntu 11.04 with Gnome and Kubuntu 11.04 with KDE. Both exhibit identical problems.
The ticking sound starts from the moment that Kubuntu boots and you hear the booting sound and until I shut down and it plays the shutdown sound. It doesn't matter whether I watch online audio/video or play my music library. I still hear the ticking/clicking. If I start and stop the song/video several times then the ticking will go away. Sometime as soon as 2 stops will make it go away sometimes I have to do it 3 to 4 times.
The second problem is, if something is playing and something else happens like an alert will pop up from the OS, or I open a menu of the player. Then the song will simply freeze in that spot, like a damaged CD and will repeat the last noise heard indefinitely.
The third problem happens mostly online, but I thought I'd post it here just in case all of these are related. Anyway, if I play a Youtube video and then click on another one. Then the second one will not have a sound. I have to reload that page and then the sound comes in.
I tried following directions on the following page:

and after copy and pasting a wget I installed gcc and copy and pasted the gcc command on that page. I received a bunch of errors and warnings and the compile aborted. After that I opened the alsa-time-test.c and it looks like an ANT/XML file not a c source file at all. So I'm sorry that I can't provide a log.
I'll be switching to Debian distribution shortly but before I do I'd like to be useful to you guys and try to provide whatever info that I can. So if someone wants to guide me through what they need I'd be happy to provide it. Please let me know. Of course the sooner the better, although I understand we are all busy.

Today is 2015 and Pulseaudio still doesn't have priopity queue, or at least prior sound device. Let me show what I mean:
For example I have 3 audio sinks (same for sources): sn1; sn2 and sn3. (I have connected them at least once, Pulseaudio knows them and keeps their list at ~/.config/pulse/sinks.list for example).
I turn on my PC, I have only sn1 connected, so it becomes default, then I connect sn2 and mark it manually as default. Now Pulseaudio knows that when sn1 is connected, sn2 is more preferable. Then I connect sn3 and mark it as default. Now sn3 has top priority. I disconnect sn3 and sn2 becomes default. Then I disconnect sn1, sn2 stays default. Then I connect sn3 and sn3 becomes default, cause it has top priority. Then I connect again sn1 and sn3 still default. It's ideal behavior for sinks and sources.
There is other option - store only one, manually selected default sink/source (seems that something like this implemented in Windows 7). How it works: I boot PC with only sn1 connected, connect sn2, amke it default manually. And now everytime I connect sn2, it will always become default, whatever devices are connected.
Why it should be implemented? Imagine gamer with USB headset. Each time he plugs it - he is to manually set it as default. module-switch-on-connect is bad workaround.

Quite often, sound suddenly starts to become totally distorted on my system. This happens quite often (but not always) when I'm watching DVB-T television with Kaffeine and pidgin IM client plays a notification sound for an incoming message.
The attached log contains a complete pulseaudio session where this bug is occuring. In the beginning kaffeine is playing fine, but near then suddenly the sound becomes distorted, and then I shut down pulseaudio.
I also attach a recording of the distorted sound coming through the speakers, made with my phone.

I am using the following version of pulseaudio:
$ sudo pacman -Q pulseaudio
pulseaudio 1.1-2
The bug manifests in the following manner. While using pidgin and having a fast conversation with a person. If there are a number of audible beeps in rapid succession the problem appears. The sound is distorted almost to a squealing state. Rarely after this happens I get 1 second of clear audio. The only way to
clear the problem is to restart the system.
Another way to have the problem manifest itself is to start totem player (I am using Gnome 3). Go to System Settings, applications and quickly slide the totem player's volume from left to right. In no time the bug manifest's itself and I am unable to use the system sounds.
If the bug manifests ALL sounds are corrupted, not just the program involved.
Let me know if there is anything else that you need to be able to fix the bug.

After switching mplayer2 config from ao=alsa to ao=pulse, sometimes playback of an audio file would stop at the end of file, without ever ending or going to next track. Manually seeking forward or back unfreezes it but it happens all the time.

I would like to see audio compression for streaming audio in the network. Especially the new codec Opus could be usefull, because it's very fast (low-latency) and efficent (good sound quality with low bitrates).
Such a feature would allow me to stream audio via wifi, what currently doesn't work with a normal tcp.sink.
