This fixes a bug. The 1.4.0-rc1 release installed the 1.3 release notes
but not any 1.4 release notes.

To fix this, let's follow the precedent set by git.  Update CMake to
install *all* release notes.

typically email me directly and send the patches as attachments. Maybe
they didn't bother to read submitting-patches.txt, so let's move it in
a more prominent location.

Rename "/doc/submitting-patches.txt" to "/HACKING.txt". No changes to
file content.

The rule lived in a strange place.  The sources for the installation
rule lived in the directory *above* the rule.

Move the rule from "$TOP/doc/CMakeLists.txt" to "$TOP/CMakeLists.txt".

Cleanup the formatting and add component for each build target.
The latter will allow us to use CPack to create a component
based installer for Windows.
Additionally install wflinfo to CMAKE_INSTALL_BINDIR rather
than hardcoding it to 'bin'.

    1. Give a basic CMake tutorial.
       (It's not Waffle's responsibility to give a half-baked CMake
       tutorial.)

    2. List all of Waffle's custom CMake options.
       (This is now covered by Options.cmake).

    3. List some important make targets
       (This will quickly become out-of-date).

Fold all headers into waffle.h except
    - The native platform headers, because users need to be able to select
      which to include and exclude.
    - waffle_version.h, which is generate at configure time.

When Waffle's documentation consisted of Doxygen in the headers, it was
sensible to maintain a separate header for each class of functions. The
Doxygen was lengthy, thus having separate headers made the documentation
easier to navigate.

Now that the documentation resides in manpage xml, each individual header
is nearly empty. It's silly to have numerous headers that declare only
3 to 4 functions each.

Strictly speaking, removing header files breaks the API. But I'm aware of
no client that will miss the removed headers.

Add a new file, /doc/html/man/index.html, that simply a list of links to
each manpge.

This will become /man/index.html on Waffle's website, and also allows
users to more easily navigate installed documentation.

The build system was copying /man/manpage.css into /doc/html/man/.
It's silly to have build rules that just copy files around, so let's just
permanently move it there.
