gallium: PIPE_QUERY_PIPELINE_STATISTICS with a single value (v3)

Here's a v3 of my last pipeline statistics series. It's largely the same, but adds a PIPE_STAT_QUERY_ALL_VALUES index which can be used to indicate that the state tracker wants all 11 values at once (the old behavior). Ilia wanted to be sure that all the old functionality was still present - and now it is - and Marek wanted to have a value indicating "all counters" - and now we do. I updated nine to continue requesting all counters for now. This value is not 0, which will break VMware's out-of-tree state trackers - but the fix is trivial. It sounded like that was OK for folks.
