What does this MR do and why?

gallium/ddebug: fixed an Segmentation fault when enable GALLIUM_DDEBUG

currently, ddebug_screen_create missing set transfer_helper member in base, so every
handle_transfer call will cause Segmentation fault if user set GALLIUM_DDEBUG enviroment
variable.
