clipping failures exposed by dEQP

Description

dEQP-GLES2.functional.clipping.polygon_edge.quad_at_origin_4,Fail dEQP-GLES2.functional.clipping.polygon_edge.quad_near_edge_2,Fail dEQP-GLES2.functional.clipping.triangle_vertex.clip_two.clip_pos_y_pos_z_and_neg_x_neg_y_neg_z,Fail

Feel free to split this bug up into more if it's a bunch of separate issues.

You can run these with

./deqp-gles2 --deqp-visibility=hidden --deqp-case=...

In all cases the results are close, just not 100% right. Usually an edge or part of an edge off.
