(pcf_read_TOC): Change counter name to avoid compiler warning.

* src/pshinter/pshrec.c (ps_hints_close): Remove redundant
declaration.
* src/pshinter/pshalgo1.c (psh1_hint_table_init): Rename variables
to avoid shadowing.
